Anxiety is a very common issue I assist clients in gaining mastery of. A foundational assumption I follow is that it is helpful to assist clients in engaging with their fears rather than denying them, or invalidating them. This understanding is supported by research in a variety of areas, including behavioral therapy, which has shown that in order to desensitize an anxious response individuals benefit from exposure so long as it is done in way that the person is not overwhelmed. From a Cognitive Therapy perspective I have also come to appreciate that very often people's anxieties are fueled by extreme interpretations and beliefs they may be having related to the fear or anxiety. From Mindfulness approaches I have come to understand the value of shifting into an observer position through observing and describing, and I have come to appreciate how this ability represents developing greater integration of the mature executive functions of the frontal lobe with the primitive survival impulses of the limbic system. The ways that I make use of these understandings with each client vary based upon the difficulties they are having with anxiety and fear, but overall I tend to support my clients in finding healthy ways to settle themselves down physiologically and then helping them to engage with their fears, the related thoughts and beliefs they have, and to not be held back from taking actions that are in their best interests based upon fears. Sometimes this involves helping clients learn meditative skills. Sometimes it involves helping them to plan out and then carry out steps in confronting fears. Sometimes it involves reprocessing traumas or anxiety producing situations in a controlled way. Sometimes it involves working with them to find new ways of thinking. Usually, in fact, I find that a combination of these is most helpful but again this is guided by each client, their situation, preferences, strengths, etc.
