Depression is not just sadness. It might be an absence of emotion, a sense of numbness, a feeling of being stuck without a chance of changing anything within one’s life. When depressed, energy just seeps away. Sometimes things matter too much, leading the mind to rehearse past wrongs and mistakes. Sometimes things do not matter at all, leading to thoughts of suicide. Cognitive behavior therapy (CBT) for depression unfolds in three steps. First, behavioural activation helps us get moving and appreciate the impact that even such small changes can have on the mood. Second, adjusting our automatic thoughts and assumptions can help us get free from painful overthinking. Finally, once we get moving a little bit and learn how to gain realistic perspective on things, we get to concentrate on deeply held life lessons (core beliefs) that make us distrust our ability to take care of ourselves and hinder our ability to create meaning in our lives. As we work on these three stages, we always pay attention to thoughts of suicide. We can only recover from depression if we stay alive. Initially, we do so by creating safety plans. As treatment progresses, life starts making sense again and thoughts of suicide dissipate.
