My approach to use of substances or compulsive behaviors (such as gambling, sex, shopping, etc.) is to view them as efforts to find pleasure or more often to avoid pain. My approach then is to understand what they client is trying to accomplish with the addiction, and then to help them find the healthiest ways to fulfil what they are attempting to accomplish. Some of my clients decide to abstain from the substance or behavior, but so too do others explore trying to manage their addiction through approach like risk reduction, controlled use, etc. I don't try to force those I work with to adhere to a particular decision about that but rather to empower them in continually evaluating the consequences of their decisions and to move toward solutions that will be healthiest for them, their loved ones, and their lives in general. Some of my clients find value in participating in 12-step recovery programs, but this is not something I require. I find am very comfortable supporting 12-step programs but so too am I also adept at supporting Rational Recovery, Smart Recovery, Mindfulness Based Relapse Prevention, Risk Reduction, Trauma Informed Addiction Treatment, and other such approaches.

Rapid detox under anesthesia. The patient remains in a private ICU room sleeping under anesthesia while our quadruple board-certified medical director, Michael H. Lowenstein, M.D., uses antagonist medications to induce and speed the acute withdrawal. The procedure lasts less than two hours, the patient wakes up shortly after and has already received medication to prevent physical cravings. The patient remains in ICU receiving IV medications, fluids, and vitamins while the team manages vitals throughout that day and overnight. Aftercare is then provided at a specialized retreat for a few days during the regulation period to maximize detox success.

Medically assisted detoxification. The patient stays in a private room for one to three days, depending on their individual needs, while receiving high levels of medication to blunt the withdrawal syndrome. The patient receives sedatives but is not put under anesthesia. This minimizes the horrible withdrawal symptoms associated with detox and maximizes comfort and safety. They also receive IV medications, fluids and vitamins while the team manages vitals. Aftercare is also provided at a specialized retreat for a few days during the process.

Aftercare services provided by Domus Retreat®. A private facility for our patients to receive continued care, around the clock support and services to successfully complete their treatment. This gives the patient the ability to recover without placing the burden of care on a family member or friend. Patients receive individual psychotherapy. This level of one-on-one attention ensures the assessment of unique emotional issues. It also enables our therapist and Clinical Director of Domus Retreat, David B. Livingston, LMFT, to suggest and guide each individual through the process of seeking further psychological or psychiatric care, when needed. Other services include massage, yoga, and counseling to support their emotional and physical transition to an opiate free life.
I am a true believer that no one intends to become an addict or alcoholic, but that one's personality, genetic makeup, and/or trauma history contribute to their drug and alcohol dependency. Depending on the client, I take either an abstinence-based or harm-reduction-model approach. I lean on the tenets of 12-step programming (such as: one-day-at-a-time; taking personal ownership; letting go of resentments) as I explore with my clients the purpose that drugs and alcohol played in their lives. While exploring or maintaining sobriety, I encourage my clients to engage with hobbies they once enjoyed, be of service to their community, and increase their social connection—all of which help to create a more meaningful life outside of their addiction. I have extensive experience working with clients who have struggled with alcohol, methamphetamines, opioids, stimulants, depressants, hallucinogens and analgesics.
